Baker MS | Free/Reduced Athletic Fees
Dear Parents & Guardians:
In Final Forms, your student-athlete’s profile had the “request for waiver” box checked off. To ensure that he/she is put on the new free or reduced lunch list for the this school year, you need to complete and return two forms to the Troy School District (TSD) Food Services department:
- The Free-Reduced Lunch application; and
- The Sharing Info with Other Programs form.

Food Services can then share your student-athlete’s free/reduced status with other programs, including athletics, and we will go into Final Forms and waive or reduce your student-athletes pay to participate fee.
Forms can be found on the TSD website at: https://www.troy.k12.mi.us/departments/food-service/free-reduced-lunch-program, or hard copies are available at the Food Services office, which is located in the Services building behind the Troy School District Central Office on Livernois. Please scan and email the completed forms to Nicole Gervais at ngervais@troy.k12.mi.us, or drop them off in person at the Food Services Dept., 4420 Livernois, Troy, MI, 48098, between 8 a.m. and 4 p.m. Monday through Friday. (Note: Food Services will only be open from 9 a.m. to 11 a.m. tomorrow, Friday, Sept. 3, and will be closed on Monday, Sept. 6, because of the Labor Day holiday.)
